Description
The overall purpose of a Shared Living Provider (SLP) is to provide residential support in the SLP’s home for individuals with developmental disabilities, under the supervision of the Residential Service Coordinator. Their primary responsibility as an SLP is to maintain a safe and healthy home environment for the individual supported. SLPs have basic home support and related responsibilities, which include providing/supporting nutritional meals; providing transportation to all appointments, to and from day services and recreational activities; providing/supporting access to community activities, cultural and religious services, if appropriate, and providing up to 24-hour supervision and supports as outline in the individuals Individual Support Plan (ISP). The daily home routine should be based on the Individuals choice but structured as determined by the Individual and ISP team which is helpful to the Individual supported in learning to function independently in a home and community setting. The SLP is responsible for teaching and supporting the Individual’s life choices, hopes, dreams, and daily documentation of that teaching. Individuals with a developmental disability are matched with trained Share Living Providers who share the same interests and hobbies as the person seeking support. The size of the home, all who live in the home (family size/kids/animals), and the location of the home are all influences on if the individual and their family are interested in meeting with a potential candidate.
The applicant process is thorough and detailed, so it depends on many factors:

- Accuracy/Completeness of Application.
- Coordination of phone and face to face interviews.
- Coordination of in-home visits.
- No criminal background, APS/CPS and Sex Offender history.
- Completion of required training.
Duties and Responsibilities:
- Implement, modify, and report on programs for individual skill acquisition according to agency policies and procedures.
- Attend to and monitor client’s needs as identified in the IPS
- Administer medication as prescribed, following doctor’s orders and utilizing the correct agency procedures.
- Ensure clients attend needed a doctor and dental appointments.
- Assist with client financial needs which may include entering transactions, establishing, and balancing accounts, ensuring that all bills are paid and budgeting income in lieu of expenses on a timely basis.
- Assist with the participants’ (clients) daily living needs which may include grooming and hygiene needs, grocery purchasing, menu planning, household management, cooking, care of clothing, etc.
- Attending to the clients’ community access needs which may include planning and organizing recreational activities within appropriate social environments, providing/arranging transportation.
- Effectively communicate as needed with parents, guardians, supervisor, and other agency staff on the status of the clients, reporting observed barriers not mentioned in the IPS to the Residential Service Coordinator.
- Act as an appropriate role model for the individuals according to the agency’s philosophy. Maintain a harmonious and harmonious atmosphere in the residential setting.
- Ability to communicate clearly, both orally and in written form
- Ability to read and understand and effectively utilize written materials and directions.
- Ability to perform basic math functions, i.e., adding subtracting, multiplying, and dividing.
- Knowledge of general home management skills with ability in the following areas: cooking, housekeeping, menu planning, household shopping, and laundry.
- Knowledge of the community and modes of access including recreational opportunities.
- General Knowledge of basic health care procedures including personal hygiene, general first aid practices, and routine dental, and medical care.
- Understanding of human development and needs on a general and individual level.
Requirements:
- Experience preferred, but not required.
- High school diploma or GED Requirements:
- Must have the ability to pass a State/Federal and APS/CPS background check (all members in the home)
- Must be at least 19 years of age
- Must have a valid driver’s license, car insurance, and current registration
- Must have a computer/laptop, printer, scanner, and internet for daily documentation.
- Must complete all required training
- Must have detailed verbal / written communication and computer skills
- Must understand and comply with agency and state policies and procedures
- Must attend and participate in all meetings
- Must maintain a clean and orderly home
- Must be able to lift 50 pounds, kneel, twist, reach, crawl, run, etc. Knowledge, Skills, & Abilities
- Certified home health aide (CHHA) preferred.
- State-approved training, as well as CPR and first aid certification.
- A minimum of 2 years’ of experience as a human services worker preferred.
- Available to work irregular hours, including night shift, weekends, and holidays.
- Extensive experience in supervising and assisting clients, as well as engaging them in suitable activities.
- Experience in monitoring clients’ conditions and administering prescribed medications.
- Ability to perform basic housekeeping tasks, prepare meals, and run errands.
- Exceptional interpersonal skills, a caring approach, and physical fitness.
- Ability to ensure clients’ safety and provide comfort.
